Test Accommodations

EBAS and PearsonVUE make every attempt to honor requests for licensees who require special testing arrangements due to a disability, including a temporary limitation and/or a physical or learning disability.

When requesting test accommodations due to a physical or learning disability, a licensee must submit a request form, a current disability report, and an evaluator review. The request form must be submitted each time an examinee applies for an examination. Each request is reviewed on an individual basis and appropriate accommodations are granted, if approved.


You must submit a test application to EBAS by either:

  1. Entering your information online and submitting it electronically to EBAS.  Click here to apply online.
  2. Printing a blank application, entering the information manually and mailing it to EBAS.

Submit a Test Accommodations Request Form (pages 3-8) of the Test Accommodations Brochure.  This document includes guidelines as well as the request form.  You may submit your test application/fees with this Test Accommodation Request Form.

Standards for test accommodations vary. Even if you have received previous test accommodation, you are not guaranteed approval of accommodations by EBAS.

Send the test accommodation request forms along with the completed application to EBAS at the address below.

Score reports may be annotated by EBAS if you take the exam with testing accommodations.

PLEASE NOTE: Licensees who are approved for test accommodations MUST contact PearsonVUE Customer Service at 
1-800-466-0450 to confirm that approved arrangements will be available for their testing appointment.
